Care Compass Network if a not-for-profit, community organization that was created to champion new models of providing the community with higher quality care, while reducing expenses through care coordination and community-focused care and education.
The CCN collaborates with more than 180 partner organizations including hospital systems, community-based organizations, nursing homes, behavioral health and substance use disorder programs, social services agencies, and similar entities.
The healthcare providers in the region have been working tirelessly behind the scenes to ensure that high quality care is accessible to residents of the Southern Tier. As part of these changes, people can expect to hear more about telemedicine, care coordinators, and behavioral services.
If your organization wants to get involved with Care Compass Network you can do so by going online or call (607) 240-2545.
CCN also wants to hear directly from residents in the community. If you have feedback that would help enhance services in the region please go online and join the “About Us” panel.
